Our school has partnered with the McMinn County Health Department to offer flu shots at school. This year, the health department will not be using paper forms. Instead, parents will complete the form online. This information goes directly to the health department, so call the health department if you have any questions, please call 423-745-7431 and speak to Tina Gallaher, RN supervisor. It is recommended that you complete the form by scanning the QR code below on a mobile device. If you prefer, you can use a browser by using the link below the QR code. You will complete the same form regardless of the way you get to it. We are happy to try this new process with the health department.
